Questions to ask while Reviewing the year:

What went bad?
Why did it go bad?
How could it not have gone bad?
What have I learned from it going bad?

Similarly

What went good?
Why did it go good?
How could it not have gone good?
What have I learned from it going good?

Do this for every important action/decision/thing that happened to you in the year. Even small daily habits which might have made a huge impact as the year progressed.
